HOW YWSS CAN SUPPORT YOU

Personal Care & Daily Living

Respectful assistance with everyday personal activities based around your individual needs, routines and preferences.

Support may include assistance with personal hygiene, dressing, grooming, toileting, eating and other daily activities identified within your NDIS plan and support arrangements.

Assistance at Home

Individualised support to help you manage everyday activities within your home and maintain greater independence.

Your support is developed around the assistance you need rather than expecting you to fit into a standard service model.

Community Access & Participation

Support to access your community, attend appointments, participate in activities, maintain social connections and pursue the things that are important to you.

Mobility & Transfers

Assistance with mobility and transfers provided by appropriately matched workers who are trained according to your individual support requirements.

Complex & High-Needs Support

YWSS has experience supporting people with significant physical disabilities and substantial daily support requirements.

We can establish dedicated teams with worker training and competencies tailored to the individual's support needs.

24/7 Support Arrangements

We have experience establishing and maintaining consistent support teams for people requiring extensive or around-the-clock assistance.

SUPPORT DESIGNED AROUND YOU

You're an individual — your support should be too.

We don't believe good disability support comes from a one-size-fits-all approach.

Before commencing services, we take the time to understand:

  • What support you need
  • How you prefer your support to be provided
  • Your routines and preferences
  • Your goals and priorities
  • The type of support workers you feel comfortable with
  • Any individual training or competency requirements
  • Who you would like involved in decisions about your support

Your support arrangements can then be developed around you and the way you want to live your life.

CHOICE OF SUPPORT WORKERS

The people supporting you matter!

Support workers can become an important part of everyday life, particularly when someone receives regular or high levels of support.

That's why YWSS believes participants should have genuine input into who provides their support.

We work with you to identify workers who are appropriately matched to your needs, preferences and support requirements.

Where appropriate, we can also discuss existing support workers who you may wish to continue working with.

Our focus is on developing stable, reliable support teams and long-term relationships wherever possible.

COMPLEX & HIGH-NEEDS NDIS SUPPORT

Experience that extends beyond everyday support

Some participants require substantially greater levels of assistance because of the nature of their disability and individual support needs.

YWSS has experience establishing and maintaining dedicated support teams for people with significant physical disabilities, complex personal-support requirements and high levels of daily assistance.

Our experience includes:

  • 24/7 support arrangements
  • Significant personal-care requirements
  • High levels of assistance with daily living
  • Mobility and transfer assistance
  • Dedicated support teams
  • Experienced disability support workers
  • AIN-qualified team members
  • Individual-specific worker training and competencies
  • Long-term workforce management
  • Collaboration with families and representatives
  • Collaboration with allied-health professionals
  • Working alongside external nursing and clinical providers

Our approach is centred on developing a stable, appropriately skilled team around the individual, while maintaining their choice, dignity, routines and preferences.

INDIVIDUAL-SPECIFIC TRAINING & COMPETENCIES

Matching skills to individual needs

Where a participant has additional or complex support requirements, having the right people is only part of the solution.

Workers may also require training and competency specific to the individual's needs.

Where appropriate, YWSS can work with relevant health and allied-health professionals to ensure workers receive appropriate training and competency assessment for the supports they are expected to provide.

This allows us to develop a support team based not only on personality and compatibility, but also on the skills and competencies required by the individual.

WORKING ALONGSIDE YOUR SUPPORT NETWORK

Good support works together.

YWSS recognises that disability support is often one part of a much larger support network.

Depending on your individual circumstances, we can work collaboratively with:

  • Family members and representatives
  • Support Coordinators
  • Plan Managers
  • Occupational Therapists
  • Physiotherapists
  • Behaviour Support Practitioners
  • Registered Nurses
  • Allied-health professionals
  • Other providers involved in your support

Our aim is to maintain clear communication and work collaboratively while keeping you at the centre of decisions about your support.
